The best probiotic for men, ranked by evidence
The answer
For the reasons probiotics are sold to men — testosterone, prostate, erections, "the male microbiome" — nothing qualifies. Not one strain, at any dose, in any trial. For four unglamorous reasons that have nothing to do with being male, specific strains do qualify, and we name them.
The outcomes that earn a grade are unglamorous, and none of them is a men's indication. Diarrhoea while taking antibiotics (Grade B), staying on H. pylori eradication therapy (Grade B), symptom relief in diagnosed IBS (Grade C), and — with heavy caveats — weight-regain prevention after dieting and LDL cholesterol (Grade C).
Where a strain does earn a grade, you usually cannot buy it at the dose that was tested. Of 27 products we took apart at label level, 25 (92.6%) have no published trial of the thing in the bottle. Of the two that do, only one was tested at the dose on its own label.

What the score does and does not measure
What it measures
One thing: how much of what you are buying the company tells you, and how much of that survives being checked against a source that is not the company. Five components, each of them a count of things printed on a label or listed on a public register, so a reader holding the bottle and a browser can reproduce every number in the table.
- Strain identity printed on the panel 24%
- Count the organisms on the Supplement Facts panel. Count how many carry a strain designator after the species name. Divide.
- Per-organism amount printed 20%
- Count the organisms on the panel. Count how many have a number of their own rather than sharing a blend total. Divide.
- What the CFU count is measured against 22%
- Three yes/no questions about the panel: is there a CFU figure, does the panel state a basis for it, does that basis reach the expiration date. Add the yeses, divide by three.
- Printed designators that lead to a human trial 20%
- Search each printed designator in quotes on PubMed. Count how many return at least one human randomised trial. Divide by the number of slots we checked.
- Certification confirmed at the issuer’s own register 14%
- Take a certification the label claims and look the product up on the certifier’s own public register. Found, or not found.
What it does not measure
- Whether the product works. A product scoring 100 has told you what is in the bottle, how much of each organism, and until when the count is guaranteed. It has not been shown to do anything.
- Anything specific to men. 0 of 294 organism slots in the founding cohort carry a strain with a human randomised trial for any male-specific outcome. Nobody in this category has evidence that would support a ranking by effect.
- Safety, purity, potency at the shelf, or value. The score reads the panel and the certifier's register. It does not open the capsule, and this site holds no laboratory data of its own.
- Rank order within a tie. Products on the same score share a rank and are left tied — 19 of them share rank 1. Every tiebreaker available here would be a number the rubric deliberately refused to weigh, smuggled back in to make a list look tidier.
The table opens on the 19 products tied at rank 1 and folds the other 135 behind a control underneath it. All 154 rows are in this page as it was served: nothing below the fold is fetched, and the fold is a stylesheet rule, so a reader with no JavaScript, a reader with no stylesheet, and a crawler all get the whole ranking.
12 further products are in the audit and not in this table. They answer less than 50% of the rubric by weight, or their label was never recovered at all, and a score built on one or two answered components presented as a summary of five would be exactly the kind of number this site exists to take apart. They are listed, unranked, with the reason, on the label audit — which also holds every column this table had to leave out.
What to buy, and what not to
Requirement of an honest answer: tell someone what to buy, or tell them there is nothing worth buying. Here is both.
Five reasons a man arrives on this page. Each one ends in something to do: a product, or nothing at all and the reason there is nothing. Each branch is summarised below in its own opening words — verbatim, never paraphrased — and links to the page that carries it in full.
The evidence, the method and the data
- Probiotics with antibiotics, for IBS, for bloating and gas, and for men over 50. The buying pages: the strains that earn a grade for each problem, which of the 166 audited labels print them, and what each panel discloses about how much of the organism is in the bottle. The antibiotics one is the only page on this site that ends in a bottle worth buying; the over-50 one compares two age-segmented products with their own brand's ordinary adult version, which is a different question and gets a different answer.
- The evidence. The 22 outcomes in one table, the strain × outcome table behind it, and 10 outcome pages carrying the trials, the effect sizes, the funding and the caveat that travels with each claim. 3 more are written and not published, and are named there rather than left silent.
- The answers. The plain questions — does it make you poop, does it move testosterone, how many CFU, is a men’s formula different from a women’s — each answered on its own page from this same fact base, verdict first and the trial behind it second.
- The methodology. How the universe was drawn, how a teardown is done, what the organism-slot denominator is, how every grade and every count was produced, and the correction policy.
- The data. The audit as CSV and JSON, with a licence and a citation line, so the table above can be checked, re-sorted or argued with off this site entirely.
- The label audit. All 166 teardowns — the 154 ranked above plus the 12 the rubric cannot score — one row per product and one column per question, against a discovered universe of 167 rows.
